Every team member taking part in the Apex Challenge must bring a completed consent form with them to register on the first day of the event. Anyone without one will not be allowed to take part.
Click on one of the links below to download a form. If you are under 18 it needs to be signed by your parent or guardian. If you are 18 or older, please fill the form in and sign it yourself. Ensure both pages are stapled together.
When you arrive at the event please wait until ALL your team is together and then bring all your consent forms to Event Centre to sign in and collect your event entry pack. It speeds things up on the registration desk if teams arrive one at a time with all of their own paperwork together, rather than as a group of three or four teams. Thanks!
